Although this seems plausible, we have some reservations about interpretation results. First, included all who had “RV dysfunction” (per criteria), yielded 180 RV out whom 145 form left (LV) (inclusive LV systolic/diastolic dysfunction) as well (Fig 1 article). Hence, it appears that majority group “biventricular” dysfunction, is unclear whether primary led to vice versa? Second, end point rates may potentially be within “early-phase” sepsis, when biventricular failure more likely prevail.2Cecconi M. Evans L. Levy Rhodes A. Sepsis-induced myocardial dysfunction: pathophysiology and management

Sepsis is aggravated by an inappropriate immune response to invading microorganisms, which occasionally leads to multiple organ failure. Several lines of evidence suggest that the ventricular myocardium is depressed during sepsis with features of diastolic dysfunction. Potential candidates responsible for septic cardiomyopathy include pathogen-associated molecular patterns (PAMPs), cytokines, a...

Pathophysiology of sepsis-induced myocardial dysfunction

Sepsis-induced myocardial dysfunction is a common complication in septic patients and is associated with increased mortality. In the clinical setting, it was once believed that myocardial dysfunction was not a major pathological process in the septic patients, at least in part, due to the unavailability of suitable clinical markers to assess intrinsic myocardial function during sepsis. Although...

[Myocardial dysfunction in sepsis].

Myocardial dysfunction appears in 25% of patients with severe sepsis and in 50% of patients with septic shock, even in the presence of hyper dynamic states. It is characterized by a reduction in left ventricle ejection fraction, that reverts at the seventh to tenth day of evolution. Right ventricular dysfunction and diastolic left ventricular dysfunction can also appear. There is no consensus i...

Deeper understanding of mechanisms contributing to sepsis-induced myocardial dysfunction

The inflammatory response of sepsis results in organ dysfunction, including myocardial dysfunction. Myocardial dysfunction is particularly important in patients with severe septic shock who progress to a hypodynamic pre-terminal phase. Multiple aspects of this septic inflammatory response contribute to the pathogenesis of decreased ventricular contractility. Inflammatory cytokines released by i...

Sepsis-induced brain dysfunction.

Systemic infection is often revealed by or associated with brain dysfunction, which is characterized by alteration of consciousness, ranging from delirium to coma, seizure or focal neurological signs. Its pathophysiology involves an ischemic process, secondary to impairment of cerebral perfusion and its determinants and a neuroinflammatory process that includes endothelial activation, alteratio...
